History – Collateral History

While the patient is usally the best source of a history, in cases such as delerium/dementia collateral history can be obtained from

1. Family, Carer, next of kin
2. Clinic Records
3. Discharge Summaries
4. ACAT assessment
5. Staff who have met patient before
6. Aboriginal health worker
7. Local pharmacy
