D&V – Diarrhoea and Vomiting

Vomiting and diarrhea are symptoms that can be caused by a wide range of conditions affecting various body systems. Using a systems-based approach, we can categorize these causes as follows:

  1. Gastrointestinal System:
    1. Infections: Such as gastroenteritis, which can be viral (e.g., norovirus, rotavirus), bacterial (e.g., Salmonella, E. coli, Campylobacter), or parasitic (e.g., Giardia).
    2. Food-Related Issues: Including food poisoning, food allergies, and intolerances.
    3. Inflammatory Diseases: Such as Crohn’s disease, ulcerative colitis, or appendicitis.
    4. Obstructions: Like bowel obstruction or severe constipation.
    5. Functional Disorders: Like irritable bowel syndrome (IBS).
  2. Nervous System:
    1. Migraines: Especially abdominal migraines in children.
    2. Vestibular Disorders: Like Meniere’s disease or motion sickness.
    3. Central Causes: Such as increased intracranial pressure or brain tumors.
  3. Endocrine/Metabolic System:
    1. Adrenal Insufficiency: Like Addison’s disease.
    2. Diabetes: Especially in the case of diabetic ketoacidosis.
    3. Thyroid Disorders: Hyperthyroidism can sometimes cause these symptoms.
  4. Infectious Diseases:
    1. Systemic Infections: Such as sepsis or viral infections like influenza.
    2. Travel-Related Infections: Like those acquired from contaminated water or food in certain regions.
  5. Toxicologic Causes:
    1. Alcohol or Drug Use: Including acute alcohol intoxication and drug side effects.
    2. Toxins: Ingestion of toxins, including certain plants, chemicals, or spoiled food.
  6. Renal:
    1. Kidney Diseases: Such as kidney infection or severe kidney dysfunction leading to uremia.
  7. Cardiovascular:
    1. Heart Failure: Sometimes, severe heart failure can cause gastrointestinal symptoms due to congestion.
  8. Hematologic/Oncologic System:
    1. Chemotherapy: Can cause nausea, vomiting, and diarrhea as side effects.
    2. Radiation Therapy: Especially when targeted near the abdomen.
  9. Psychological Causes:
    1. Eating Disorders: Such as bulimia nervosa.
    2. Stress and Anxiety: Can manifest as gastrointestinal symptoms in some individuals.
  10. Pregnancy-Related:
    1. Morning Sickness: Common in early pregnancy.
    2. Hyperemesis Gravidarum: Severe nausea and vomiting in pregnancy.
  11. Pediatric Considerations:
    1. Intussusception: In infants and young children.
    2. Milk Protein Allergy or Lactose Intolerance: Particularly in infants and toddlers.
  12. Autoimmune Disorders: Such as celiac disease.
  13. Miscellaneous:
    1. Postoperative: Following surgery, particularly abdominal surgery.
    2. Side Effects of Medications: Numerous medications can cause nausea, vomiting, and diarrhea.

It is important to note that vomiting and diarrhea can be symptoms of a wide range of conditions, and their cause should be determined based on the individual’s overall clinical picture, including their medical history, concurrent symptoms, and potential exposure to infectious agents or toxins. In many cases, especially when symptoms are severe, persistent, or accompanied by other worrying signs like dehydration, medical evaluation is necessary to determine the cause and appropriate treatment.
